Breast Prostheses

Silicon (silicon) is the most abundant natural element on Earth (25.4%) after oxygen. The form suitable for medical use is included in many brands and models of prostheses. A prosthesis consists of a silicone gel in the center and a sheath using different technologies such as a silky surface layer, silicone layer or polyurethane foam layer.

They are produced in different volumes, shapes (drop or round) or surface properties (smooth surface / rough surface silicone, silky surface, polyurethane foam layer).

 

Since breast prostheses are foreign names, they cause a capsule reaction when implanted in the body, just like tooth, knee, hip prostheses or intra-abdominal materials used in general surgery. Although the development of this capsule is an expected reaction, it can rarely be seen that the capsule formed due to the breast prosthesis contracts painfully, compresses the prosthesis and causes the prosthesis to change shape. Much more rarely, malignant cells called anaplastic large cell lymphoma can be seen around the capsule.

 

In both cases, the prosthesis and capsule should be removed, sent for pathologic examination and replaced with new prostheses.

After the Operation: Depending on the technique used, you may not feel any pain or you may have mild pain that passes with painkillers. In submuscular applications, this pain may be of a character that restricts your arm movements for a few days.

The special bra or breast band worn after the operation is used for about 8 weeks.

Breast Augmentation

It is the process of giving the breast a fuller form using silicone implants. While the breast volume is increased, it is also lifted at the same time. It is an operation with immediate results.

 

It can be planned for women who are unhappy with insufficient breast volume as a result of insufficient development of the milk glands or decrease in milk and fat glands such as weight loss after breastfeeding.

 

The operation is performed under general anesthesia through a suitable incision and the volume is increased by selecting a prosthesis according to the patient’s wishes and the suitability of her body.

Am I suitable for this surgery?

If you use a padded bra and are unhappy with your breast shape and size, any woman over the age of 18 is suitable for this operation. 

I am planning to have a child, will it affect my breastfeeding? Does silicone pass into milk? Will I harm my baby?

Since the prosthesis is mostly placed under the breast muscle envelope (subfascial) or directly under the muscle (submuscular), it does not affect your breastfeeding. In some literature studies, an increase in the amount of silicone excreted in milk was found in some patient groups, but no statistically significant difference was found when comparing the levels of silicone in commercial cow's milk and ready-made infant formula. In addition, there is no information that micro doses of silicon in the blood cause genetic anomalies in the fetus.

What kind of breast will I have after surgery? Can I choose the size prosthesis I want?

Which prosthesis shape is better? (Drop/ Round)

Both prostheses have their own advantages and disadvantages. Depending on the breast tissue placement, the amount of subcutaneous fat tissue and your wishes, the appropriate prosthesis shape is selected. Your surgeon will inform you about this in more detail.

I don't want scars, where will the implant be placed?

The prosthesis can be placed around the nipple, under the breast or in the armpit. Since recent studies have shown that access routes other than under the breast may have a negative effect on capsular contracture, it is recommended not to use them unless necessary.

Will my implant be placed under the milk gland?

If your skin thickness and desired size are appropriate, it is more appropriate to place the implant under the muscle envelope (subfascial) due to its advantages such as ease of breast examination and protection of the pectoral muscle, especially in professional athletes who use the pectoral muscle. However, depending on your expectations and the suitability of the body structure, dual plan or subfascial plans can be applied.

Is the surgery painful? Is there a risk?

As with any surgical procedure, complications related to anesthesia or surgery may occur at varying rates. These include prosthesis displacement, bleeding, infection, capsular contracture and anaplastic large cell lymphoma, which has been on the agenda in recent years. They are very safe operations with painless pain on the first day and rapidly decreasing pain in the following days.

Is there a risk of my prosthesis exploding?

In the old generation prostheses, such problems can be seen as a result of the saline fluid in the silicone sheath escaping as a result of tearing or leakage, while such a problem does not develop in the new generation prostheses produced with today's technology.

Breast Reduction

It is an operation to reduce breast volume by removing fat and milk glands together with different techniques in hypermastia (breast size), which is mostly familial. 

 

Breast size discomfort, which causes bra strap cuts, shoulder and neck pain, under-breast rashes, may be genetically inherited or due to weight gain. In this case, it is more appropriate to first think about healthy weight and then plan breast shaping operations. A method I use for breast volume reduction and breast shaping is liposuction. Especially laser liposuction is very useful in shaping the chest walls of selected cases. At the end of the operation, your scar may be in the shape of a lollipop, J, mini inverted T or large inverted T. Since most of the scar will remain under the breast fold, it is not noticeable unless there are wound healing problems. As in Game operations, the healing quality of your wound in breast reduction operations is greatly affected by your genetic structure, diet, smoking and health conditions such as diabetes.

After the Operation: Mostly drains are not used. However, if it is necessary to use them to remove the blood and fluid accumulated in the breast, they will be painlessly removed within a few days. Contrary to popular belief, breast reduction surgeries are much more painless surgeries. After the surgery, you will start to use a special bra that you will use for about 8 weeks, you can return to your social life in a few days, and you can start doing light sports after 1 month.

Are there risks to the operation?

Like all surgical interventions, there are complications related to anesthesia or surgery in this operation with variable frequency published in the literature. These may include wound healing problems, bleeding disorders, infection, fat necrosis, skin rashes, areola loss or deformities. The risk of complications increases as breast volume and sagging increase, as well as in the presence of risk factors such as concomitant chronic diseases and smoking.

I plan to have a baby. When should I have surgery?

If you are planning to have a baby in the near future, it would be better to postpone your surgery. Because the tissue we have reduced will grow again with pregnancy hormones and your breast will sag and the aesthetic form we have gained will deteriorate. In addition, breastfeeding may be impaired after surgery, albeit very rarely.

I am very overweight. Should I have this surgery after losing weight?

If you tend to lose weight, it is more appropriate to have this surgery during the plateau phase when your weight loss stops and the appropriate weight is reached. However, if you are unable to lose weight, you can have this surgery beforehand to increase your exercise capacity and your motivation to lose weight.

Can I breastfeed my baby after the operation?

Since approximately seventy percent of the milk gland and nipple sensation is preserved, you can breastfeed your baby.

However, depending on the technique to be used, the volume of tissue to be removed and the physiology of milk secretion, the ability to lactate may vary.

Which technique suits me?

The appropriate technique for you is chosen according to your breast volume, the location of your nipple on the chest wall, your skin quality, your lifestyle, your general health status and your expectations.

If the distance between the pit in the middle of your collarbones and the nipple is greater than 28 centimeters, the technique that leaves an inverted T-shaped scar is usually chosen. If the distance is less than 28 centimeters and your skin quality is good, the vertical reduction technique that leaves a lollipop-shaped scar is more suitable for you. In cases where the patient's age is advanced or accompanied by chronic diseases such as blood pressure, diabetes, heart disease, obesity, techniques that will complete the operation quickly are used.

Will my breasts grow again after surgery?

Milk glands are hormone-sensitive tissues, and when the hormone level increases during pregnancy or for different reasons, your breasts will enlarge as there will be an increase in breast volume. Similarly, with weight gain, the fat tissue component of the breast will also increase, which may increase your breast volume. However, when the hormonal effect is removed or when you lose weight, your breasts will shrink again.

I have a relative with breast cancer, can I still have this surgery? Does it increase the risk of breast cancer?

No, on the contrary, since breast tissue will decrease, the breast tissue where breast cancer cells can settle will also decrease. However, mammography and breast magnetic resonance imaging (MRI) should be routinely performed under the age of 40 instead of preoperative breast ultrasound. These radiologic images will serve as a kind of base map for your breast and postoperative imaging will help to distinguish physiologic / natural changes in breast healing from pathologic / diseased ones.

both of my breasts are different from each other, will they be different after the surgery?

It is very natural for both breasts to be different from each other. After the operation, tissues of different volumes are removed from the breasts to ensure equality. After the operation, this difference will disappear or become very small.

Breast Lift

It is the process of lifting the breast that has sagged due to the effect of gravity and deterioration of skin quality from where it is located on the chest wall over time. It can be performed with only a lifting procedure or by placing a silicone prosthesis. 

 

It is a suitable procedure for patients who have complaints about the shape and sagging of their breasts rather than their increased volume. As a result of sagging caused by excessive weight changes, the effect of pregnancy gravity and deterioration in skin quality, the cleavage that creates a feminine image is emptied. With this surgery, the volume can be increased with the prosthesis in line with the patient’s expectations, while at the same time the enlarged nipple can be moved to a higher position by narrowing it.

What kind of scar will I have?

The scar of breast lift surgery depends on its degree. In breasts without excessive skin sagging and with good skin quality, lifting surgery with a prosthesis placed around the nipple may be sufficient. However, in cases with severe skin and breast tissue sagging, a lollipop scar or inverted T scar may remain after lifting.

Can I breastfeed again after this surgery?

Although there is no harm in repeating the operation, I recommend that you postpone this surgery if you are planning a pregnancy in the near future. The surgery does not affect your breastfeeding, but the milk glands that we suspend will grow with the effect of pregnancy hormones and will cause new sagging and nipple enlargement, which will disrupt the newly acquired breast form.

How long after breastfeeding can I have this surgery?

These surgeries can be planned 4-6 months after breastfeeding.

What are the risks? Will it heal painfully?

Breast lift operation without augmentation is a short-term surgical procedure with a very safe and rapid recovery.

However, poorly healing scars, numbness or asymmetry in the nipple may be seen due to environmental factors such as genetic structure, smoking, vitamin/mineral balance, UV.

I also want it to grow in size. Is that possible?

This can be achieved with a silicone prosthesis with a volume that meets your expectations suitable for the width of the rib cage, but the size of the prosthesis should be carefully selected as the gravity effect will increase as the size of the prosthesis to be used increases.

Male Breast Aesthetics Gynecomastia

Breast enlargement due to an increase in the volume of milk glands or fat tissue in male patients is called gynecomastia. It is a common condition affecting 50-60% of adult men. It can be corrected with classical surgical gynecomastia correction methods or combined methods with modern devices.

 

It mostly develops as a result of excess fat tissue and skin due to excessive weight gain, but it can also be seen in patients taking medication (estrogen, anti androgen, anabolic steroid cimetidine, spironolactone-containing drugs), active sports or thin patients. Since ovarian diseases and hormone tissue disorders may be among the causes, Kaan also recommends preoperative examination with imaging methods. Open surgical tissue removal, laser lipolysis and liposuction methods can be used in patients with breast size complaints lasting longer than one year.

Is my 14-year-old son suitable for this surgery?

Gynecomastia is usually seen with the increase in hormonal effect from the age of 10 and peaks at the age of 13-14. However, studies have shown that only 10% of these complaints continue at the age of 18. Therefore, the operation should not be performed in patients younger than 18 years of age or in the presence of newly noticed complaints that have not completed one year.

What is lipomastia (pseudogynecomastia)?

It is often misdiagnosed as gynecomastia. The difference from gynecomastia is that breast size is caused by excess fat tissue rather than an increase in the milk gland.

How will the operation take place?

It can be performed under general anesthesia or sedation. Depending on the stage of gynecomastia, classical open tissue excision technique or liposuction methods combined with laser surgery can be used.
